Admissions
If you or someone you know is in need of our rehabilitation and nursing services, or is interested in our hospice or respite care options, our social services team can answer the questions you may have. We will be happy to inform you of the admissions requirements as they relate to each of the different levels of care that we offer, and will work with you in open communication to determine the most appropriate level of care for you or your family member. We are certified to accept patients with Medicare, Medicaid, managed care, and private funds, and our team will work with you to explain the financing options available. We can also provide you with information regarding private and semi-private room availability, wait list information, and placement options. For hospitalized persons, our admissions coordinator will establish a direct communication with the patient's hospital discharge planner to ensure timely admission to NHC when the patient is ready for subacute or rehabilitative care.
Upon admission, we gather a social history to make the patient's environment as full of familiar activities as possible, including a number of social functions that are coordinated with our Recreation department. Our social workers will have ongoing communication with patients and families in order to efficiently assist with discharge planning when the patient's status has changed for need of other options, whether that be home, assisted living, home health, or traditional nursing care.
Our social services team is here to ensure that you and your family are well informed from admission to discharge. We have established strong relationships with area hospitals, home health agencies, and other groups to ensure that a well connected continuum of care is provided for our patients and community.
